Here are some additional forms that will be needed in the next few
weeks.

Parent/Player Conference form will have to be signed and turned in
after the 3 Feb Meeting. Players will have to have this on file to play.

Athletics Permission Form will need to be on file at the school in order
to play.

The School Medication Prescriber/Parent Authorization Form will need
to be filled out if the player needs any medication at all (to include
tylenol, advil) while with the team and a parent is not available to
administer. Also the Overnight Field Trip Medical Release Form will
need to be on file to go to out of town tournaments.

Coach Arndt’s Medical Release Form he keeps in his Briefcase is
included.

Enclosed is the Overnight Field Trip Med Release Form which
has been sent out by email to returning players
